Contest Brief

I would like to create a Logo & Microsoft Word Header/Footer for:
Platinum Health Solutions Group.
I have a link to my pinterest board I've created to help depict the mood and feel I'm after. I would like a HEXAGON shape as the logo.

Header and Footer is to have a logo on the top and can possibly have the name at top or bottom - depending on design lay out.
I would like to keep the blue colouring.
