Contest Brief

I need a covid-19 screening application workflow concept map. the idea of the app is that it connects to the students' database of the school, after the student's sign in, the app ask takes him to the questioning screen that asks multiple questions about his health, if the student doesn't have any health issues related to covid-19, the student will receive something like a green card with a barcode on it that can be authenticated by the management of the school. If the student has any health issues related to covid-19, the app will give them a red card of message for example and asks students to go to nearest hospital and doesn't need to come to school.
The app should also notify the management of the school that this exact student received a red flag.

I attached images of an app the has a similar idea, but please don't drew a concept of the same thing.
